Colorado drivers now have another option when choosing license plates at the DMV.  During the legislative session that ended in early May 2022, the state legislature passed a law creating a new hospice and palliative care license plate intended to raise awareness of these important services and directly support hospice and palliative care education. The new hospice & palliative care license plate debuted on July 1, 2022. Show your support for hospice & palliative care families and your co-workers by placing the new plate on your vehicle (motorcycles included).

A $50 donation to COCHPC secures your certificate to take to the DMV.  A portion of the cost benefits the Colorado Center for Hospice & Palliative Care, a nonprofit organization that supports hospice education. The other proceeds from these license plates will fund awareness and education initiatives including professional development for health care workers who support hospice and palliative care patients, as well as public campaigns aimed at connecting more families and their loved ones with end-of-life care.

After you've completed your donation through the form below, simply print the certificate (confirmation email) that our system creates for you and take it to the DMV to get your hospice & palliative care license plate. The certificate will be emailed to you upon submitting your donation through the form below. 

You do not have to be an HHAC member or work at a hospice to have the hospice & palliative care license plate.
